Implementing the P.R.I.D.E. model and the Family-Centered Case Management Model of Practice, you will offer a range of services aimed at safeguarding, preventing, and assisting children and their families.
In this position, you will:
- Assume responsibility for case management and resource allocation concerning an assortment of initiatives aimed at supporting children in care, foster families, and extended family caregivers.
- Advocate for and provide constructive assistance to regional endeavors that consistently advance or augment the provision of services for children and families.
- Conduct training sessions or organize group gatherings to provide education or support.
- Perform family assessments on potential foster, adoptive, or family-based caregivers.
- Assist in the execution of placements and offer ongoing support and maintenance thereafter.
- Offer assistance to caregivers in formulating strategies that prioritize the well-being of children and in securing essential resources.
- Construct and organize the child’s reunification in the care of foster parents or other responsible parties.
- As required, act as a liaison and consultant with FNCFS and other community agencies concerning decisions concerning children and the provision of services to caregivers.
