The Seniors’ clinic employs a hybrid model of care to ensure that all of our resources are accessible to seniors with complex medical histories. Our medical staff sees patients for Consultation (e.g., Geriatric Assessments) and Primary Care. If a patient is unable to leave their home, we will visit them at their residence.
Using the social model and a team-based approach, you will provide safe, high-quality patient care in both clinical and community settings. You will collaborate with the Seniors’ Clinic team to assist patients in safely aging at home. The team’s collaborative approach ensures that patients have access to all relevant interprofessional team members and community resources. Utilizing initiative and sound judgment, you will carry out a variety of nursing tasks as the doctor or designated clinician directs in order to ensure the comfort and quality of patient care.
You have graduated from an accredited nursing program and are currently registered to practice as a Registered Nurse. A BScN and a certificate in gerontology are preferred. CPR and first aid training must be current. Work experience with the elderly and the disabled. Computer literacy is required.
You are an outstanding communicator who can facilitate effective leadership, team building, change, and resource management. As an outstanding role model, you can facilitate mentoring and collaboration. In addition, you exhibit clinical competence, problem-solving, decision-making, and conflict-resolution abilities.
