Position Summary

The PCBH Provider will collaborate with the medical home team to establish a comprehensive support and service system within the medical home of the patient. What Does the Role Do?

• In order to better identify, triage, assess, and provide behavioral strategies and support to primary care patients presenting with a variety of mental health and physical health concerns, the PCBH Provider will receive extensive training in the PCBH model.
• The PCBH Provider shall employ concise evidence-based treatment interventions and best practices, such as motivational interviewing, cognitive behavioral strategies, and motivational education, to formulate patient-centered plans for behavior change.
• This position entails delivering care to families, groups, and patients of all ages, stages, and abilities.
• The position requires collaboration, education, and consultation with external and internal healthcare teams, as well as other referral and community organizations.
• Additional care needs that are in line with the patient’s identified treatment objectives may be necessary, as they fall within the purview of PCBH and the professional responsibilities of the registered nurse or social worker.
• You will be expected to successfully assimilate into the culture and tempo of Medical Home co-location, with the assistance of coworkers from both inside and outside the organization.

Who Should Apply for Primary Care Registered Nurse Jobs in Calgary?

• Social work, Registered Psychiatric Nurse, or Bachelor of Science in Nursing.
• Enrollment in the appropriate college, which is authorized by the Health Professions Act.
• Present BLS (Basic Life Support) for the medical professional.
