Position Summary

The PCBH Provider will collaborate with the medical home team to establish a comprehensive support and service system within the medical home of the patient. What Does the Role Do?

  • This position entails delivering care to families, groups, and patients of all ages, stages, and abilities.
  • The position requires collaboration, education, and consultation with external and internal healthcare teams, as well as other referral and community organizations.
  • Additional care needs that are in line with the patient’s identified treatment objectives may be necessary, as they fall within the purview of PCBH and the professional responsibilities of the registered nurse or social worker.

Who Should Apply for Primary Care Registered Nurse Jobs in Calgary?

  • Social work, Registered Psychiatric Nurse, or Bachelor of Science in Nursing.
